Our Care Management Programs address a number of high-risk and complex conditions, including Congestive Heart Failure, Diabetes and Asthma just to name a few. Care Managers work with members and their families, Primary Care Physicians, facilities and other service providers to assess, plan, coordinate, monitor and evaluate the optimal mix of services to best meet the needs of each member.
